Insurance and Practice Policy

Insurance Accepted

We are contracted providers with the following health insurance companies listed.

Dr. Hadi, Dr. Raza, and Ms. Azra Husain accept the following:

  • (BCBS) Blue Cross Blue Shield- PPO (Excluding Blue Choice and BCBS HMO)
  • (NMPN) Northwestern Medicine Physician Network (Delnor HMO)
  •  Aetna-PPO
  • Cigna- PPO

Ms. Jennifer Milan accepts the following:

  • (BCBS) Blue Cross Blue Shield- PPO (Excluding Blue Choice and BCBS HMO)
  • (NMPN) Northwestern Medicine Physician Network (Delnor HMO)

Important
If your insurance needs a referral from your primary care provider, please make sure you bring a brief referral note with you to the clinic.

For NMPN carriers, please note that you intake process of pre-certifications/ prior authorizations at your end while we cater to responsibilities at our end.

Appointments

  • Please call (630)- 770-3475 to schedule an appointment along with any questions and concerns you may have. If we happen to miss your call please leave a brief message with your name and contact number.
  • Our staff will call to verify your insurance and proceed with a brief telephone intake in form of simple questions to understand your needs.
  • One additional step of confirming your appointment might be necessary if your insurance requires additional steps for verification.
  • As we evolve with technology advancements, we will offer options to access portal of EMR in order to complete paperwork.
  • On the day of the appointment, please arrive 15 mins early with a photo ID, Insurance cards, completed paper work etc.

Cancellation and No shows

  • If you are a new patient and do not keep your appointment you will be charged in full!
  • We require a 24-hour notice for cancellations and to reschedule all appointments.
  • Late cancellation is when you cancel within 24 hour
  • Charges:
    Cancellations of Doctor’s follow up visit: $150.00
    Cancellations for the Therapists Appointments are: $120.00

Payments

  • We accept payments in the form of cash, checks (Payable to Cafal Clinic), credit cards (Visa and Mastercards) and the co-payment is due at the time of the service.
  • Out of network payment: (Eg. United Healthcare)
    • We have a fixed rate of payment that is due at the time of service. You will be provided with an invoice that shows diagnostics codes and procedure codes which you can use to get reimbursed.

Communication:

  • Please keep up with using the patient portal or other forms of means as we evolve with technological advancement.
  • Crisis: Please go to the nearest Emergency Room or call 911 in the case of a life-threatening emergency. You can call Northwestern Hospital (Cadence) at (630)- 933-4000 to schedule an intake evaluation to determine the level of care needed such as in-patient or day programs.
  • Urgent calls: Will be returned within 24 hours. Please make sure you leave a clear detailed message.
  • Routine call backs: This is usually during the clinical days which is Monday, Wednesday, Friday, and every 2nd and 4th Saturday of the month.

Refill Requests

Our goal is to minimize refill requests. Follow up appointments must be scheduled at least a week before you run out of your medication. Please call the office for a refill and DO NOT have the pharmacy fax this request!

Forms and Letters

  • All medication forms and letters for school need a 48-hour notice. We would need the name of the school nurse and a DIRECT fax number.
  • The Family and Medical Leave Act (FMLA) including any disability forms require a 7 to 14 business day notice. All forms have to be accompanied by essential information necessary including time and exact dates. Charges apply.
